แนวทางการให้รหัสทางตา

01. ต้อกระจก ให้เลือกรหัสตามลำดับดังนี้

• ถ้ารู้สาเหตุของต้อกระจก ให้รหัสตามสาเหตุ โดยไม่คำนึงถึงอายุของผู้ป่วยหรือตำแหน่งความขุ่นของเลนส์ เช่น Q120 Congenital cataractH261 Traumatic cataractH263 Drug induced cataract
• ถ้าไม่สามารถบอกสาเหตุของต้อกระจก ให้ใช้อายุเป็นเกณฑ์อายุน้อยกว่าหรือเท่ากับ 40 ปีให้รหัส H260 Infantile, juvenile and presenile cataractอายุมากกว่า 40 ปี ให้รหัส H250-H259 Senile cataract ซึ่งแบ่งตามตำแหน่งและลักษณะ ความขุ่นของเลนส์ ถัาเป็น Combined opacity เช่นมี Nuclear sclerosis(NS) with posterior subcapsular opacity(PSC) ให้รหัส H258 Combined forms of senile cataract

02. ในผู้ป่วยเบาหวาน มีโรคแทรกซ้อนทางตาที่พบบ่อยคือ ต้อกระจกและเบาหวานเข้าจอประสาทตา ให้รหัสควบดังนี้คือE103+(Insulin dependent DM with ophthalmic complication) H280*(Diabetic cataract)E113+(Non insulin dependent DM with ophthalmic complication) H360*(Diabeticretinopathy)

หมายเหตุ Diabetic cataract ระยะแรกมีลักษณะเป็น superficial anterior and posterior subcapsular cortical opacity เมื่อเป็นมากขึ้นจะเป็น multiple grey white subcapsular opacity (snowflake appearance) หรืออาจพบเป็น bilateral wide spread subcapsular lens change, abrupt onset in uncontrolled young diabetic patient มักพบ diabetic cataract ในผู้ป่วย DM ชนิด insulin dependent ผู้ป่วย DM มักจะเป็น Senile cataract เร็วกว่าคนปกติ พบเป็นชนิด nuclear sclerosis(NS) with posterior subcapsular opacity(PSC) ให้รหัสดังนี้Principal Dx. H251 Senile nuclear cataract ถ้าเป็น NS cataractหรือ H258 Combined forms of senile cataract Comorbid E119 Non insulin dependent DM without complication

03. ภาวะสายตาผิดปกติ (Refractive error) ผู้ป่วยมักมีภาวะหลายอย่างร่วมกัน ให้ใช้หลักเกณฑ์ในการเลือกรหัสดังนี้

• ถ้าค่าของ Astigmatism ที่พบร่วม น้อยกว่าหรือเท่ากับค่าของ Myopia หรือ Hypermetropia ให้รหัสเป็น Myopia หรือ Hypermetropiaท ถ้าค่าของ Astigmatism ที่พบร่วม มากกว่าค่าของ Myopia หรือ Hypermetropia ให้รหัสเป็น Astigmatismเช่น +3.00D -2.50D x180' ให้รหัส H520 Hypermetropia-2.50D -1.00D x180' " H521 Myopia-1.00D -3.00D x180' " H522 Astigmatism
• ถ้าภาวะสายตาผิดปกติในตาสองข้างไม่เหมือนกัน (ดูจากค่าของ sphere) เช่น ตาขวา - 1.00D -1.00D x180' ตาซ้าย + 1.00D -1.00D x180' ค่าของ sphere ต่างกันไม่เกิน 2.50Dให้รหัสเป็น H526 Other disorders of refraction (Antimetropia)
• ถ้าภาวะสายตาผิดปกติในตาสองข้างไม่เท่ากัน (ดูจากค่าของ sphere) และมีค่าต่างกันมากกว่าหรือเท่ากับ 2.50Dเช่น ตาขวา - 2.50D -1.00D x180'ตาซ้าย + 1.50D -1.00D x180' ค่าของ sphere ต่างกัน 4.00Dหรือตาขวา - 5.50D -1.00D x180'ตาซ้าย - 2.50D -1.00D x180' ค่าของ sphere ต่างกัน 3.00Dให้รหัสเป็น H523 Anisometropia and aniseikonia
• ถ้าผู้ป่วยภาวะสายตาผิดปกติ มาตรวจเพราะปัญหาของ PresbyopiaPrincipal Dx. H524 PresbyopiaOther Dx. H52x แล้วแต่ชนิดของ refractive error

04. ผู้ป่วยกลุ่มอาการ ICE syndrome ไม่มีรหัสเฉพาะ ให้ใช้รหัสตามสิ่งที่ตรวจพบเช่น Progressive iris atrophy ให้รหัส H212 Iris atrophy (essential)(progressive)Chandler's syndrome ให้รหัส H182 Other corneal edemaCogan Reese'syndrome ให้รหัส H212 Degeneration of iris and ciliary body

05. การทำ Pterygium excision with beta irradiation ให้รหัส 1131 (Simple pterygiumexcision) with 9521 (Superficial radiation)

06. การทำ Pterygium excision with MMC ให้รหัส 1131 (Simple pterygium excision)with 9925 (Injection or infusion of antineoplastic agent)

07. Corneal graft rejection or failure จากสาเหตุของ inflamation or infection ให้รหัสPrincipal Dx. T868 Failure or rejection of corneal graftsAdditional Dx. Z947 Corneal transplant statusถ้าเป็นจากสาเหตุอื่นให้รหัสPrincipal Dx. T853 Mechanical complication of corneal graftAdditional Dx. Z947 Corneal transplant status

08. ผู้ป่วยมาตรวจหลังการผ่าตัด ตามแพทย์นัด ให้รหัส Z090 (Follow up examinationafter surgery for other condition) จนกระทั่งไม่จำเป็นต้องให้การรักษา เช่นFollow up 1 week post PE with IOL ให้รหัส PDx. Z090 Follow up 1 month post PE with IOL ให้รหัส PDx. Z961 (Pseudophakia) เมื่อไม่จำเป็นต้องให้ยาหยอดตา

09. ผู้ป่วยหลังการผ่าตัดต้อหิน หลังจากหยุดยา anti-inflamation eye drop แล้ว ความดันตาอยู่ในระดับปกติ โดยไม่จำเป็นต้องใช้ anti-glaucoma drug เมื่อมาตรวจตามนัด ให้รหัสโรคเดิมเป็น Principal Dx. ตามด้วยรหัส Z เช่นPrincipal Dx. H401 Primary open angle glaucomaAdditional Dx. Z488 Other specified follow up care (to deal with residual stateor to prevent recurrence)

10. ผู้ป่วยหลังการผ่าตัดตาเหล่ ใช้หลักการตามข้อ 09

11. ผู้ป่วยที่มา admit เพื่อทำผ่าตัด แต่ไม่ได้ทำให้รหัสกลุ่ม Z เพื่อบอกสาเหตุที่ไม่ได้ทำเช่น Principal Dx. H250 Senile incipient cataract Additional Dx. Z530 Procedure not carried out because of contraindicationE115 DM with diabetic ulcer

12. ผู้ป่วยที่มี complication จากการผ่าตัด ให้ใช้รหัสกลุ่ม T เพื่อให้รายละเอียด เช่น ทำPE with IOL with dropped nucleus ให้รหัสPrincipal Dx. H250 Senile incipient cataractComplication T818 Other complications of procedures, NECProcedure 1341 (PE) with 1371 (IOL implantation)
